NASA formed an independent panel to review the failure of the tethered satellite experiment and the loss of the satellite. NASA selected Kenneth J. Szalai, Director of Dryden Flight Research Center, to chair the panel. The formation of such an investigative body in the wake of a costly loss conformed to standard practice under NASA's Space Flight Operations Contingency Plan. In a NASA press release announcing the panel's formation, Szalai stated, "given the public investment in the tethered satellite, it is important that we fmd out what went wrong."
